SIYAA Universal Documents
The South Iredell Youth Athletic Association (SIYAA), in an effort to streamline the process of organizational management and required documentation from all participating member schools, teams, coaches and players, we have created the documents below. They have been voted on by the executive board and any changes/amendments from year to year will typically be included and noted within the individual document.
As a reminder, the Acknowledgement of Concussion Information for Parents & Players must be reviewed and signed by the athlete and their parents/guardians prior to the beginning of first practice. In addition, every coach must complete the computer based training (CBT) found at www.cdc.gov/HEADSUP prior to the beginning of their first practice. When the CBT is completed, the coach will print out a copy of their certificate. The certificate will have their name and date on it to validate its currency. Once coaches have completed the training, it is good for one year from the date on the certificate. We will have to have a certificate on hand for everyone or they cannot coach. It is a requirement for all sports from the county.
All Coaches MUST COMPLETE the Concussion Training Course found at www.cdc.gov/HEADSUP before they hold their first practice. (Required for ALL Sports)
